Automatic Payment and Deposit Inventory

Use this form to gather information about all the automatic and other electronic payments/deposits that will need to be changed to/from your new account at WGSB. We recommend looking back at 3 months of account statements. You will also want to include any recurring/annual payments and renewals you may have.

Complete this form to notify employers, government agencies, and others of your new account for future payroll and electronic payments.

Use this form to let companies and other vendors know that you have a new account.

Authorization to Close Account

Once your account is opened and all electronic payments/deposits have successfully switched to your new account, give the old bank permission to close your account. This form will provide them with the necessary information and signature(s).
